Botulinum toxin is naturally produced by a specific type of bacteria. However, commercial sources are manufactured artificially and the product contains no bacteria. Although it is the same toxin that causes the rare disease known as botulism, the amounts used in cosmetic surgery are so tiny that it is very safe to use for this purpose. The toxin works by temporarily paralysing or weakening the muscle into which it is injected. Weakening the muscle minimises the overlying skin lines and wrinkles that occur because of muscle contraction. The commonly targeted areas include the forehead, crow's feet around the eyes, and neck lines. The aim is to produce a smoother, younger looking facial appearance.
Botulinum toxin is branded in two main trade names: BOTOX ® and DYSPORT®. They are produced by different manufacturers although the product is essentially the same.
Botox (or botulinum toxin) injections are gaining in popularity and its use is now widespread and almost commonplace. As well as its cosmetic uses, there are some other specialised applications, such as reduction in axillary (armpit) sweating and the treatment of migraines and of muscle spasms.
We administer botulinum toxin using a very small needle and syringe directly into the target muscle. A local anaesthetic is usually not required principally because injecting the anaesthetic can be more uncomfortable than the treatment itself. Occasionally, we may use topical local anaesthetic creams in advance to minimise any discomfort. The injections are given carefully to avoid the risk of infection and bruising.

Botulinum toxin stings a little as it is injected, however this sensation does not last. The effects do not occur immediately but rather with a gradual onset after 48 hours, reaching a peak at one to two weeks later. It is a temporary treatment which wears off after several weeks to months. The average duration of effect is three months. With repeated injection however, the effect may become more prolonged.
